PDE5 inhibitors (tadalafil, sildenafil, vardenafil)
- Do not take with nitrates (nitroglycerin, isosorbide) or recreational nitrites ("poppers") — combination can cause severe, potentially fatal low blood pressure.
- Use caution with alpha-blockers (doxazosin, tamsulosin) and other blood-pressure medications — can cause lightheadedness or fainting.
- Not appropriate if you have had a recent heart attack, stroke, or severe cardiovascular disease, or have uncontrolled hypertension or hypotension.
- Seek emergency care for an erection lasting more than 4 hours (priapism) or sudden vision or hearing loss.
- Common side effects: headache, flushing, mild nasal congestion, back pain, indigestion. Usually mild and transient.
Apomorphine (dopamine agonist)
- Can cause nausea — most patients take an antiemetic on the first few uses until tolerance develops. Discuss with your prescriber.
- Can cause sudden lightheadedness or fainting, particularly when standing up quickly. Sit or lie down if symptoms occur.
- Do not combine with dopamine antagonists (most antipsychotics, metoclopramide) or MAO inhibitors.
- Use caution if you have a history of syncope, Parkinson's disease treated with other dopaminergics, or low blood pressure.
